The aim of the proposed research is to devise techniques for supporting objective medical decision making in relation to insulin-dependent diabetic patients. The decision support system to be developed will be capable of providing advice on longer-term strategic issues of diabetic management such as changes in the basic insulin regimen that are prescribed at 3 or 6 monthly visits to the clinic; this contrasts with the majority of existing approaches that focus on shorter-term issues such as day-by- day insulin dosage adjustment. The approach adopted will involve both qualitative and quantitative reasoning which will be incorporated in a system which consists of three inter-related modules; a blood glucose data processing module; a knowledge-based module providing qualitative advice on the basis of the processed data; and a dynamic patient simulator, incorporating a quantitative physiological model of carbohydrate metabolism. Together they will provide an integrated decision support environment for assisting a physician, or diabetic specialist nurse in the management of insulin-treated diabetic patients. The research programme comprises both development and testing of the proposed decision support prototype.
