A reliable and practical method to assess the performance of the cerebral autoregulation mechanism is urgently needed for clinical management of newborns and adults at risk of cerebrovascular complications. Current proposals require an externally imposed large disturbance in arterial blood pressure (ABP) and are not suitable for patients undergoing intensive care. We have shown that it is possible to estimate the status of cerebral autoregulation using undisturbed baseline recordings of Doppler ultrasound and ABP in newborns [Paneria et al., Stroke, 1995; 26: 7480]. The present proposal aims at evaluating this method in a larger population of newborns, including patients with intraventricular haemorrhage, asphyxia and hydrocephalus. More than 400 recordings are available from 110 neonates and data collection will continue for cases of birth asphyxia. We will attempt to advance towards the prototype of a dedicated system for diagnosis and bedside monitoring of neonatal and adult patients at risk of cerebrovascular complications by improving the performance of the coherent average technique and finding better ways to assess the status of cerebral autoregulation.
