Minimally invasive tumour treatments, including high intensity focused ultrasound (HIFU) and laser-based thermal ablation are currently being used clinically, but are limited by the need for improved image guidance and monitoring methods. We propose to develop three novel ultrasound techniques for improving the clinical effectiveness of these therapies. Backscatter temperature imaging (BTI) visualises changes in sound speed that result from reversible heating and will be developed for the targeting the treatment volume prior to tissue damage. Reflex transmission attenuation imaging (RTI) and elasticity imaging (EI) will be developed for monitoring the growth of the region of tissue necrosis. Computer simulations and studies of phantoms and HIFU - or laser treated excised tissue will be used to develop image reconstruction algorithms, to measure performance, to develop methods for overcoming problems and artefacts, and to develop display methods for combining the images with 3D conventional ultrasound images. A system for clinically evaluating the new 3D images will be constructed and tested in the laboratory, in normal volunteers and as part of an ongoing clinical trial of HIFU treatment of liver metastases.
