We aim to develop an enhanced Fluorescence Detection System (FDS) to improve the detection of early gut cancers by using tumour fluorescence as a diagnostic aid. Patients at high risk of developing cancer undergo regular endoscopies but many early cancers are not visible with commercially available equipment. Our previous work has shown the viability of fluorescence detection. This new project represents a significant advance of the technique both technically and in its clinical application.The FDS will comprise a custom designed endoscope (designed by Storz to our design) coupled to a twin cameras and a novel spectrometer for spectral finger-printing of suspicious lesions. A more powerful and spectrally optimised light source will excite brighter fluorescence. Sophisticated image processing will give a single image containing all the clinical information.5-ALA will be used as the photosensitiser, as it is rapidly excreted and causes no serious side-effects. Patients with known cancers will be tested first to validate the system, then the FDS will be used to identify cancers in patients with pre-cancerous colon polyps, and finally used during colonoscopy in patients at high risk.A FDS has the potential to improve early tumour diagnosis resulting in significant therapeutic benefits.
