In image guided surgery MR or CT images are registered to the physical space of the operating room assuming the validity of the rigid body transformation. These images are then used for navigation during the surgical procedure. We propose to extend this concept in two important ways to permit image guidance in soft tissues. Firstly we will devise a methodology to align multiple, arbitrarily orientated ultrasound images to a CT volume. Ultrasound slice orientation will be determined using a 3D localiser. Secondly we will devise methods to compensate for the motion and distortion of soft tissues during the intervention.We will apply this new technology to percutaneous photocoagulation of liver metastases using pre-operative CT guidance by intra-operative ultrasound. Metastatic spread to the liver has, until recently, usually been fatal. New therapies are showing potential to prolong life and perhaps even cure some patients. This work will combine the advantages of interactivity and low cost of ultrasound with the better diagnostic performance of CT and other imaging modalities. We propose that this has the potential to improve the efficacy of ablation of liver metastases, extend the number of patients who might benefit from this procedure, and lower costs.
