Informatics has the potential to revolutionise healthcare practice, by allowing information on individual patients to be shared seamlessly and by facilitating the systemisation of medical knowledge. This assumes that the relevant information can be captured efficiently and represented unambiguously. The NHS makes a huge investment in imaging investigations but at present the main output is normally a radiologists textual report, which is not machine interpretable. As a result, information is often used inefficiently in the management of individual patients, and is virtually unusable for data-mining and knowledge systemisation. Attempts to provide electronic reporting have generally been unsuccessful. The reporting process is slower than the conventional approach and expressiveness is limited. The solution is to develop more intelligent systems that understand what the user is trying to do, reducing the need for interaction. If the system is to understand the user, and allow natural interaction, this requires the integration of several technologies: speech input, automated image analysis, medical terminology analysis, and knowledge representation. We plan to develop a prototype integrated reporting system and assess it using two pilot reporting tasks.
